Influence of storage time of leukocyte depleted erythrocyte suspension on quality of washed red blood cells

摘要: 目的:通过对不同储存期去白细胞悬浮红细胞(去白悬红)制备的洗涤红细胞中钾离子浓度(K+)、游离血红蛋白含量(FHb)以及溶血率的检测,了解去白悬红的储存时间对制备洗涤红细胞质量的影响。方法:收集储存期1周(1 w组) 、2周(2 w组) 、3周(3 w组)、4~5周(4~5 w组)的去白悬红各10袋,手工制备成洗涤红细胞,分别于制备后立即(0 h)和(4±2)℃保存24 h取样测定其K+、FHb,并计算溶血率。结果:洗涤后0 h,4组K+含量分别为(0.38±0.04)、(0.44±0.08)、(0.53±0.23)、(0.65±0.20)mmol/L,3 w组和4~5 w组明显高于1 w组、2 w组(P<0.05);洗涤后保存24 h,各组K+分别为(1.89±0.32)、(1.89±0.60)、(2.02±0.45)、(2.62±0.31)mmol/L,4~5 w组明显高于前3组(P<0.05),但均低于人体正常K+上限。无论是洗涤后0 h,还是保存24 h,FHb、溶血率4~5 w组明显高于前3组(P<0.05),。而且4~5 w组洗涤红细胞保存24 h,FHb为(0.90±0.32)g/L,明显高于国标水平。结论: 建议尽量避免使用保存期4~5 w的去白悬红制备洗涤红细胞,而且制备好的洗涤红细胞应尽快输注,减少红细胞在生理盐水中的悬浮时间。Abstract: Objective:To study the influence of the storage time of leukocyte depleted erythrocyte suspension on the quality of washed red blood cells by testing the concentration of potassium (K+), free hemoglobin (FHb) and calculating hemolysis rate. Method:10 bags of leukocyte depleted erythrocyte suspension preserved for 1 week (1 w group), 2 week(2 w group), 3 week (3w group) and 4-5 week (4-5 w group) were chosen to prepare washed red blood cells manually, and samples were taken to measure (K+), FHb and calculate the hemolysis rate at the time 0 h and 24 h after preparation in (4±2)℃. Result:The (K+) of washed RBCs at 0 h after preparation of 1 w group, 2 w group, 3 w group and 4-5 w group were (0.38±0.04), (0.44±0.08), (0.53±0.23) and (0.65±0.20)mmol/L, respectively, and 3 w group and 4-5 w group were significantly higher than that of other two groups (P<0.05). And at 24 h after storage the (K+) were (1.89±0.32), (1.89±0.60), (2.02±0.45) and (2.62±0.31)mmol/L, respectively, and 4-5 w group were higher than that of other groups (P<0.05). But all the (K+) were still lower than the upper limit of (K+) in normal human bodies. The FHb and the hemolysis rate of washed RBCs at 0 h and 24 h after preparation of 4-5 w group were significantly higher than those of the other groups (P<0.05). And the FHb of washed RBCs at 24 h after preparation of 4-5 w group was (0.90±0.32) g/L which was significantly higher than national standard. Conclusion:Leukocyte depleted erythrocyte suspension at the end of storage should be avoided to prepare washed RBCs and the prepared washed RBCs should be transfused as soon as possible to reduce the suspension time in saline solution.
