摘要: 目的:探讨血友病假肿瘤患者的临床特征及影像学诊断、治疗和转归。方法:对9例血友病假肿瘤患者的临床特征、影像学表现、处理和转归等资料进行回顾性分析。结果:9例患者均为血友病甲(4例重型,5例中型),均有外伤史。主要影像学表现为骨质疏松,破坏,软组织肿胀。2例患者超声显示为巨大血肿,块内分隔,部分呈低强回声,包块上呈均匀强回声,或低回声占位,包膜完整,边缘不规则,内有分隔,表现为低回声占位。6例行手术治疗和替代治疗基本治愈。3例行FⅧ因子替代治疗,其中1例无改善自动出院,1例好转,1例治愈。结论:影像学结合病史可以明确诊断血友病假肿瘤,手术结合足够的替代治疗是安全有效治疗假肿瘤的手段。

Abstract: Objective: To explore the clinical diagnosis and management of hemophiliac pseudotumor.Method: The clinical feature,image findings,management and outcome of 9 patients with hemophiliac pseudotumor were analyzed.Result: All patients were hemophilia A (4 severe cases and 5 moderate cases).All patients sustained an injury prior to the development of pseudotumor.Main image findings included osteolysis lesion,soft tissue swelling or lump.Six patients received surgical therapy and achieved remission.Three patients only accepted replacement therapy and one patient achieved improvement of swelling.Conclusion: Hemophilic history of patients can contribute to the right diagnosis of pseudotumor.Surgery combined with sufficient replacement therapy is safe and effective.
