摘要

摘要: 目的：总结和分析p表型孕妇的妊娠管理办法，确保母婴健康。方法：搜索中国知网、万方医学网，PubMed数据库，查阅并借鉴p表型孕妇的妊娠管理措施，对云南省曲靖市第一人民医院收治的1例p表型孕妇进行妊娠管理。结果：本例孕妇未经过特殊治疗，成功产下足月儿，新生儿有轻微的新生儿溶血病（HDN），经过蓝光照射等措施，新生儿好转出院。结论：IgG4性质的抗-PP1Pk抗体可通过胎盘导致HDN，该HDN症状较轻，孕妇妊娠期不需要预防性干预治疗。可根据孕妇抗体效价、IgG抗体亚型、胎儿游离DNA A4GALT测序、MCA-PSV等，对p表型孕妇进行妊娠管理。

Abstract: Objective: To summarize and analyze the methods of p-phenotype pregnant women to ensure the health of mothers and babies.Methods: By searching the CNKI, Wanfang Medical Network, PubMed database, consulting and learning from the pregnancy management measures of p-phenotype pregnant women, one pregnant woman with p phenotype was admitted to the First People's Hospital of Qujing City, Yunnan Province for pregnancy management.Results: The pregnant woman in this case successfully gave birth to a full-term baby without special treatment. The newborn had mild HDFN. After blue light irradiation and other measures, the newborn was discharged from the hospital.Conclusion: The anti-PP1 Pk antibody of IgG4 nature can cause HDN through the placenta. The symptoms of HDN are not so serious and preventive intervention is not needed during pregnancy. Pregnancy management can be performed on pregnant women with p phenotype according to the titers of pregnant women's antibodies, IgG antibody subtypes, fetal free DNA A4GALT sequencing, MCA-PSV.
