摘要: 目的:探讨血清白/球蛋白比值(A/G)与初诊多发性骨髓瘤(MM)患者临床指标的相关性及预后意义。方法:回顾性分析2014年1月至2020年10月我院血液科收治的142例初诊MM患者的临床资料及生存情况,通过受试者工作特性曲线分析得出最佳截断值,根据A/G水平分为低比值组(A/G≤0.54,83例)和高比值组(A/G>0.54,59例)。采用χ2检验比较2组间临床特征的差异,应用Kaplan-Meier法及Log-rank检验计算和比较生存率,采用Cox风险比例模型进行单因素和多因素分析。结果:A/G高比值组与A/G低比值组间年龄、DS分期、ISS分期、β2-微球蛋白比较,差异有统计学意义(P<0.05)。单因素分析结果显示,年龄、DS分期、ISS分期、β2-微球蛋白、A/G是MM患者的不良预后因素(P<0.05)。Cox多因素分析提示,年龄、DS分期、ISS分期、A/G是MM患者预后不良的独立危险因素(P<0.05)。结论:初诊时A/G比值低下提示预后不良,是MM患者独立的预后因素。Abstract: Objective: To explore the correlation between serum albumin-globulin ratio(A/G) and clinical indicators in newly diagnosed multiple myeloma(MM) patients and its prognostic value.Methods: The clinical data and survival status of 142 newly diagnosed MM patients admitted to the Department of Hematology in our hospital from January 2014 to October 2020 were retrospectively analyzed. The best cut-off value was obtained according to the receiver operating characteristic curve analysis. The patients were divided into low ratio group(A/G ≤ 0.54, 83 cases) and high ratio group(A/G>0.54, 59 cases) based on the A/G level. Differences of clinical characteristics between the two groups were compared by using χ2 test. Kaplan-Meier method and Log-rank test were used to calculate and compare survival rates, and Cox risk ratio model was used for univariate and multivariate analysis.Results: There were significant differences in age, DS staging, ISS staging and β2-microglobulin between the high ratio group and the low ratio group(P<0.05). The results of univariate analysis showed that age, DS staging, ISS staging, β2-microglobulin and A/G were the poor prognostic factors of MM(P<0.05). Cox multivariate analysis suggested that age, DS staging, ISS staging and A/G were independent risk factors for poor prognosis in MM patients(P<0.05).Conclusion: A low A/G ratio at the first diagnosis indicates a poor prognosis, which is an independent prognostic factor for MM patients.
