摘要: 目的:研究经血小板滤器去除手工浓缩血小板中的白细胞后对血小板的质量及体外功能的影响。方法:从新鲜全血中制备手工浓缩血小板,用床边型白细胞过滤输血器滤除白细胞,分别用细胞计数、离心、血小板质量检测技术及流式细胞术等方法,测定过滤前后血小板计数、白细胞计数、血小板压积、平均血小板体积、大血小板比率、血小板分布宽度、CD62p表达率、血小板黏附、血小板聚集和低渗休克等指标,并观察临床输注反应。结果:经床边型血小板滤器除去白细胞后,浓缩血小板回收率为90.32%,白细胞去除率为98.21%。除血小板计数和白细胞计数有明显下降外,血小板压积、平均血小板体积、大血小板比率、血小板分布宽度、血小板黏附和聚集、低渗休克反应和CD62p表达率均无统计学意义。临床血小板输注不良反应发生率明显降低。结论:浓缩手工血小板去除白细胞,过滤对血小板功能无明显影响,血小板回收率及剩余白细胞数符合相关要求,可以明显降低临床输注反应的发生。Abstract: Objective:To explore the effect of leukocyte filtration on in vitro function of platelet concentrates. Method:Platelet concentrates was prepared from fresh whole blood, then filtered with bedside filters. The counted numbers of platelet and leukocyte, PCT, MPV, P-LCR, PDW, CD62p expression, platelet aggregation, platelet adhesion and hypotonic shock response were detected before and after filtering. Result:After filtering leukocytes, the platelet recovery and leukocyte removal rate was 90.32% and 98.21%, respectively. Besides the WBC and PLT counts were declined obviously, PCT, MPV and PDW, P-LCR shock response, platelet adhesion, aggregation and hypotonic shock response and CD62p expression rate had no statistical significance. Transfusion reaction significantly decreased after filtering leukocytes. Conclusion:Leukocyte filtration would do not affect the platelet function in vitro. The platelet recovery and leukocyte removal rate were in accordance with the requirements. Leukocyte removal could significantly reduce the incidence of transfusion reactions.
