Difference analysis of treponema pallidum-specific antibodies detection between domestic Kemei chemiluminometer and imported Abbott chemiluminometer

摘要: 目的:以梅毒螺旋体免疫印迹法(TP-WB)为金标准,比较科美和雅培2种化学发光仪在检测梅毒特异性抗体时的差异。方法:选取科美检测梅毒阳性样本80例和雅培检测梅毒阳性样本80例,160例样本分别用2种化学发光检测梅毒特异性抗体后再用WB方法复检。结果:科美CLIA阳性预测值为90.07%,阴性预测值为88.89%,符合率为90.00%。雅培CMIA阳性预测值为93.19%,阴性预测值为100.00%,符合率为93.75%,两者差异无统计学意义(χ2=0.950,P=0.330;χ2=1.513,P=0.219;χ2=1.507,P=0.220)。当科美S/CO>9.01时阳性预测值为100%,雅培S/CO>6.01时阳性预测值为100%,含有P47特征条带的样本数为108例(78.83%),为所有特征条带中出现频率最高。结论:科美和雅培化学发光在检测梅毒时与WB法比较均有较高符合率,雅培S/CO值≥ 6.01、科美S/CO值≥ 9.01时检测结果即为真阳性S/CO值,临床工作中可以使用价格较低廉的科美CLIA作为雅培CMIA的有效补充。Abstract: Objective: To compare the difference of two chemiluminescence analyzers in detecting specific antibodies to syphilis, using Treponema pallidum immunoblotting(TP-WB) as the gold standard.Method: Eighty cases of positive syphilis positive samples detected by Chemimetric and 80 positive cases of syphilis positive samples detected by Abbott were selected respectively.Syphilis-specific antibodies of the 160 samples were detected using Kemei CLIA and Abbott CMIA chemiluminometer, and retested by WB method.Result: The positive predictive value, negative predictive value and coincidence rate of Kemei CLIA were 90.07%, 88.89% and 90.00%, respectively.The positive predictive value, negative predictive value and coincidence rate of Abbott CMIA were 93.19%, 100.00% and 93.75%, respectively.There was no significant difference between the two types of chemiluminometer(χ2=0.950 P=0.330, χ2=1.513 P=0.219, and χ2=1.507 P=0.220).When Kemei S/CO>9.01 the positive predictive value was 100%, when Abbott S/CO>6.01 the positive predictive value was 100%, and 108 samples had the P47 feature band, accounting for 78.83%, which was the highest frequency occurred in all feature bands.Conclusion: Kemei CLIA chemiluminometer and Abbott CMIA chemiluminometer might have comparatively high coincidence rate with WB method when detecting syphilis.The test results would be true positive S/CO values when the Abbott S/CO value is more than 6.01, and the Kemei S/CO ratio is more than 9.01.In clinical work, Kemei CLIA can be used as an effective supplement to Abbott CMIA because of its lower price.
