摘要: 目的 了解无偿献血人群中RhD阴性血型血清学表型分布特点。方法 选择中心血站2017年1月—2022年8月共39 105名初次献血者中RhD血型初筛为阴性的141例样本,采用血型血清学检测方法进行确认试验,利用血型群体遗传学研究方法进行描述性分析。结果 141例RhD血型初筛阴性献血者中,确认RhD阴性139例、RhD变异2例,RhD阴性献血者占献血人群的0.36%;139例确认RhD阴性献血者4种主要抗原频率分别为C:0.446 0,c:0.935 3,E:0.057 6,e:1.000 0,RhCcEe表型分布特征为ccee>Ccee>CCee>CcEe=ccEe,其中ccee表型和Ccee表型占87.8%,表型频率观察值与期望值比较差异无统计学意义(χ2=5.327,P>0.05);139例确认RhD阴性献血者中,ABO血型分布为A型(40.3%)、O型(33.1%)、B型(20.1%)、AB型(6.5%)。结论 RhD阴性献血人群中,4种主要抗原表达以e抗原频率最高,RhCcEe表型分布以ccee、Ccee为主,ABO血型分布中A型占比最高、O型次之。Abstract: Objective To investigate the distribution of RhD-negative serological phenotype among the voluntary blood donors.Methods The 141 samples which initial screening RhD-negative were collected from 39 105 first-time blood donors from January 2017 to August 2022 at Central Blood Station. The confirmation test was conducted by serological methods, and descriptive methods were used for studying blood group population genetics.Results Among 141 first-time RhD-negative blood donors, 139 cases were confirmed as RhD-negative, and two cases were confirmed as RhD-variant, the frequency of RhD-negative blood donors was about 0.36%; the frequencies of four major antigens in 139 RhD-negative blood donors were C: 0.446 0, c: 0.935 3, E: 0.057 6, e: 1.000 0, the distribution of RhCcEe phenotype was: ccee>Ccee>CCee>CcEe=ccEe, and the proportion of ccee and Ccee was 87.8%, there was no significant difference between the observed and the expected numbers(χ2=5.327035, P>0.05). Among 139 RhD-negative blood donors, ABO blood groups distribution was type A(40.3%), type O(33.1%), type B(20.1%) and type AB(6.5%).Conclusion Among RhD-negative blood donors, the most frequently expressed of the four major antigens was found that e antigen, the majority of RhCcEe phenotype were ccee and Ccee. In the distribution of ABO blood types, the proportion of A blood group was the highest, followed by O blood groups.
