Detection before blood transfusion and safe blood transfusion of autoimmune hemolytic anemia patients

摘要: 目的：探讨自身免疫性溶血性贫血（AIHA）患者输血前血的实验室检测，选择合适的血液品种，保障患者输血安全。方法：对2011-2012年本院49例AIHA患者输血前实验检测，接抗人球蛋白试验，氯喹放散试验检测自身抗体掩盖的不规则抗体，用乙醚放散试验检测自身抗体血型特异性。结果：49例患者中间接抗人球蛋白试验阳性者15例，检出不规则抗体5例。自身抗体具有血型特异性者于间接抗人球蛋白试验阳性者中检出4例，间接抗人球蛋白试验阴性者中未检出。结论：AIHA患者输血前实验室检测，尤其检测被自身抗体掩盖的不规则抗体及鉴定自身抗体的血型特异性对患者安全、有效的输血具有重要的意义。

Abstract: Objective: To discuss blood detection before blood transfusion for the autoimmune hemolytic anemia (AIHA) patients,in order to choose appropriate blood components and ensure blood transfusion safety.Method: From 2011 to 2012,49 AIHA patients took before blood detection before transfusion,irregular autoantibody covered by autoantibody was detected by indirect antiglobulin test and chloroquine radiation test,and autoantibody blood group specificity was detected by ether radiation test.Result: Among 49 cases indirect anti globulin test positive was 15 cases,irregular antibody positive was 5 cases.There were 4 positive of autoantibody with blood type specificity in the cases of indirect anti globulin test positive,and not found in the cases of indirect anti globulin test negative.Conclusion: The blood detection before blood transfusion for AIHA patients,especially irregular autoantibody covered by autoantibody detection and identification of autoantibody blood group specificity,would be of important significance for safe and effective blood transfusion. -
