应用毛细管电泳法和高效液相色谱法对血红蛋白定量的对比研究

Comparison of automated capillary zone electrophoresis and high-performance liquid chromatography on quantitative study of hemoglobin

  • 目的:探讨用法国Sebia公司Capillarys2全自动毛细管电泳(CZE)对血红蛋白定性与定量分析的准确性。方法:采集205例广东地区成人EDTA-K2抗凝血2 ml,分别采用高效液相色谱法(HPLC)和capillarys方法各自测定血红蛋白含量。结果:两种方法测定HbA2的准确度,在205例样本中,CZE测定有93%的测定结果与HPLC测定值的百分差异在±20%范围内,与95%相比较,P值为0.1944,差异无统计学意义。其中CZE测得的HbA2异常21例,HbF增高5例,Hb A2+HbF同时增高12例;用HPLC对这205例样本进行复检对照,结果基本一致。检测出HbH 7例,HbE 7例,Hb NewYork 5例,Hb G-thailand 3例,Hb Bart's 3例,HbS 2例。但是,对于HbA2的定量,用CZE测得的水平高于HPLC。同时,我们观察到CZE较HPLC对HbH和Hb Bart's的检测更为敏感。结论:HPLC和CZE都能能够分离和量化出HbA2和HbF异常增高,是一种快速、准确、可靠的筛查地中海海贫血实验方法,但是两者在鉴定异常血红蛋白时存在区别。
