摘要: 目的:探讨GAS6在儿童下呼吸道感染性疾病和脓毒症诊断中的应用价值。方法:分别检测240例疾病组和55例正常对照组血清GAS6、PCT和CRP水平。比较各疾病组和正常对照组之间GAS6、PCT、CRP的差异。同时,绘制各诊断指标的ROC曲线。结果:脓毒症组、细菌性肺炎组和病毒性肺炎组患儿的血清GAS6均显著高于正常对照组(P<0.05),但支原体肺炎组患儿的GAS6与正常对照组比较差异无统计学意义(P>0.05)。脓毒症组患儿血清GAS6显著高于另外3组(P<0.01),此外,细菌性肺炎组患儿血清GAS6明显高于支原体肺炎组和病毒性肺炎组(P<0.05),但支原体肺炎组和病毒性肺炎组患儿间并无统计学差异(P>0.05)。无论诊断脓毒症还是细菌性肺炎,GAS6的ROC曲线下面积均小于PCT和CRP(P<0.05),但诊断病毒性肺炎时,GAS6与PCT的ROC曲线下面积差异无统计学意义(P>0.05)。结论:在儿童下呼吸道感染性疾病中,患儿血清GAS6、PCT和CRP会显著升高。但GAS6对儿童下呼吸道感染性疾病及脓毒症的诊断价值不及PCT和CRP。Abstract: Objective:To evaluate the diagnostic value of GAS6 in children with lower respiratory infection diseases and sepsis.Method:A total 240 hospitalized children including 54 subjects with sepsis,65 subjects with bacterial pneumonia,60 subjects with mycoplasma pneumonia,61 subjects with viral pneumonia and 55 normal controls were recruited in our study.The concentration of serum GAS6,PCT and CRP were assayed in the five groups.The values of serum GAS6,PCT and CRP were compared between the group of sepsis,bacterial pneumonia,mycoplasma pneumonia,viral pneumonia and normal control.Meanwhile,the ROC curves with GAS6,PCT and CRP were provided by the SPSS.Result:The serum concentration of GAS6 in the groups of sepsis,bacterial pneumonia and viral pneumonia were significantly higher than that of normal control group(P<0.05),but the GAS6 level showed no significant differences between the mycoplasma pneumonia and normal control groups(P>0.05).The GAS6 of patients with sepsis were significantly higher than that of patients with bacterial pneumonia,mycoplasma pneumonia and viral pneumonia(P<0.01).In addition,the GAS6 of patients with bacterial pneumonia was higher than that of patients with mycoplasma pneumonia and viral pneumonia(P<0.05),but the GAS6 level showed no significant differences between the viral pneumonia group and mycoplasma pneumonia group(P>0.05).Comparing the AUC of each index in diagnosis of sepsis and bacterial pneumonia,the AUC of GAS6 was less than PCT and CRP(P<0.05),but the AUC of GAS6 and PCT showed no significant differences in the diagnosis of viral pneumonia(P>0.05).Conclusion:The concentration of GAS6 in children patients with lower respiratory infection diseases and sepsis was significantly elevated,and it could not distinguish the pathogens in children patients with lower respiratory infection diseases.The PCT and CRP might have more diagnostic value than GAS6 in children patients with lower respiratory infection diseases and sepsis.
