Correlation between expression level of plasma miR-34a and prognosis of acute lymphoblastic leukemia

摘要: 目的:分析急性淋巴细胞白血病(ALL)血浆微小核糖核酸-34a (miR-34a)表达水平与预后的相关性。方法:回顾性分析2016年12月—2018年12月收治的112例ALL患者资料,作为观察组,同时根据疗效标准分为完全缓解(CR)组40例、部分缓解(PR)组55例和未缓解(NR)组17例,并选取同期进行体检的健康者100例作为对照组,检测血浆miR-34a表达水平,比较不同临床特征及疗效患者的血浆miR-34a水平,并采用Kaplan-Meier法及多因素Cox回归模型分析血浆miR-34a水平与ALL患者预后的相关性。结果:观察组血浆miR-34a水平低于对照组(P<0.05);miR-34a低表达的ALL患者白细胞、乳酸脱氢酶、骨髓母细胞比例及危险度分型高于miR-34a高表达的ALL患者(P<0.05);CR组血浆miR-34a水平与对照组比较差异无统计学意义(P>0.05),PR组血浆miR-34a水平低于CR组及对照组(P<0.05),NR组血浆miR-34a水平低于其余各组(P<0.05);miR-34a低表达的ALL患者总生存率低于高表达患者(P<0.05);多因素Cox回归分析显示,白细胞、骨髓原始细胞、危险度分型及miR-34a是总生存率的主要危险因素(P<0.05)。结论:ALL患者血浆miR-34a水平呈低表达,且与预后密切相关。

Abstract: Objective: To analyze the correlation between the expression level of plasma microribonucleic acid-34 a(miR-34 a) and prognosis of acute lymphoblastic leukemia(ALL).Methods: A retrospective analysis was performed on the clinical data of 112 patients with ALL(observation group) admitted to the hospital between December 2016 and December 2018. The patients were divided into complete remission(CR) group(n=40), partial remission(PR) group(n=55) and non-remission(NR) group(n=17) according to the criterion of therapeutic effect. Meanwhile, 100 healthy individuals were selected as the control group. The expression level of plasma miR-34 a was detected, and the plasma miR-34 a levels of patients with different clinical characteristics and different curative effects were compared. Kaplan-Meier method and multivariate Cox regression model were used to analyze the correlation between plasma miR-34 a level and the prognosis of patients with ALL.Results: The plasma miR-34 a level in observation group was lower than that in the control group(P<0.05). The white blood cell, lactic dehydrogenase, bone marrow blast cell ratio and risk classification of ALL patients with low miR-34 a expression were higher than those of ALL patients with high miR-34 a expression(P<0.05). There was no statistically significant difference in plasma miR-34 a level between CR group and the control group(P>0.05). PR group had lower plasma miR-34 a levels than CR group and the control group(P<0.05), and NR group had lower plasma miR-34 a levels than the other groups(P<0.05). The overall survival rate of ALL patients with low miR-34 a expression was lower than that of patients with high expression(P<0.05). Multivariate Cox regression analysis showed that the white blood cell, bone marrow initial cells, risk classification and miR-34 a were main risk factors influencing overall survival(P<0.05).Conclusion: Plasma miR-34 a level is low in patients with ALL, and it might be closely related to the prognosis.-
